To be responsible for implementing an effective medicine management system within the practice, including cost-effective prescribing, working as an independent prescriber, providing advice and support for both patients and colleagues. In addition, the post-holder will also support the multi-disciplinary team, in line with the strategic objectives of the practice.

Main Duties of the Role

a.    Practice as an independent prescriber working within scope of practice

b.    Carry out face-to-face and telephone consultations with patients with minor illnesses

c.     To provide medication review services with patients in the practice and during domiciliary visits to the local nursing home

d.    To provide advice and support for patients with self-limiting conditions

e.    Encourage cost-effective prescribing within the practice

f.      Review all safety alerts ensuring the required actions are taken

g.    To reconcile medicines following patients discharge from hospital or clinic appointments

h.    Review medications for newly registered patients

i.      Implement and embed a robust repeat prescribing system

j.      To provide subject matter expertise on medication monitoring, implementing and embedding a system

k.     Carry out targeted medication reviews for high-risk patients

l.      To reauthorise medication for repeat prescribing within scope of practice

m.   Identify patients suitable for repeat prescribing, liaising with the relevant clinicians as required

n.    Review and implement safe prescribing systems for vulnerable patients, improving compliance

o.    Support clinicians with the management of patients suffering from drug and alcohol dependencies

p.    Actively signpost patients to the correct healthcare professional

q.    Contribute to practice targets (QAIF etc.), complying with local and regional guidance

Job Description

Responsibilities include promoting cost‑effective prescribing, reviewing safety alerts, reconciling medicines after hospital or clinic discharge, and reviewing medications for newly registered patients. The role also requires implementing a robust repeat prescribing system, providing expertise on medication monitoring, delivering targeted reviews for high‑risk patients, reauthorising repeat medications, and identifying patients suitable for repeat prescribing in collaboration with clinicians. Additional duties include strengthening safe prescribing systems for vulnerable patients, supporting clinicians in managing drug and alcohol dependencies, signposting patients appropriately, and contributing to practice targets such as QAIF in line with local and regional guidance.

About Riverside Medical Practice

Riverside Medical Practice is based at Pontllanfraith Health Centre and is a busy single site practice with a stable list size of approx 8800 patients. You will work alongside an excellent clinical team comprising of 4 GPs, 1 Advanced Nurse Practitioner, 2 Practice Nurses, 1 Health Care Assistant, 1 phlebotomist, and an excellent receptionist and administrative team. We also have a team of NCN/HB Allied Health Professionals working from the practice site to include a Physiotherapist, a Psychological Wellbeing Practitioner and a Pharmacist. The District Nursing Team are also based within the building along with other Health Board Allied Health Professionals and an Independent Dispensing Pharmacy also provides a service from within the building. We provide an exceptional service and are proud to have maintained have been awarded 'Best GP Practice in South Wales - 2024' and runners up in 2023. The practice has a manageable workload and serves a friendly practice population with a strong emphasis on community values.
